Age Waiver Form
Instructions
YLM Age Division Waivers
- Players wishing to play up one age-division (e.g., 12U playing up to 14U) may do so by right if approved by their association subject to its policies and procedures regarding play-ups.
- Players must not play up more than one age division (e.g., 10U to 14U) without express permission from the YLM.
- Regardless of YLM age-waivers, players must register in accordance with USA Lacrosse Age Segmentation and must ensure proper age-verification documentation under USAL's membership policies.
If a player wishes to play down in YLM league play (e.g., 12U to 10U), the following steps must be completed:
1. Player must obtain approval from local association subject to its policies and procedures regarding play-up/play-down.
2. Player should register with local association according to date of birth and verified USAL age policy.
3. Association leader, coach, or parent should complete YLM age waiver form by clicking the link below.
4. YLM will notify the parent and appropriate board members of the local association in due course if YLM has any concerns about the age waiver. (YLM response time will vary depending on timing and volume.)
Generally, play-down waivers will be appropriate for players asking to play down with other players in their cohort (grade level) and within 90 days of the USAL age-segmentation guidelines. YLM will, however, evaluate all play-down requests on a case-by-case basis.
